Purpose
Upperforms is a form builder. We want it to be used for legitimate data collection. This policy defines what's allowed and what will result in account suspension or termination.
Allowed Uses
- Customer feedback and satisfaction surveys
- Event registration and waitlists
- Lead generation with clear disclosure to respondents
- Employee onboarding and HR forms
- Research surveys with proper informed consent
- Order forms and intake questionnaires
- Any other lawful data collection with respondent consent
Prohibited Uses
You may NOT use Upperforms to:
Deception & Fraud
- Create phishing forms designed to steal credentials or sensitive data
- Impersonate other people, companies, or brands
- Collect credit card or financial data without PCI-compliant infrastructure
- Deceive respondents about how their data will be used
Harmful Content
- Collect data from minors under 13 without verified parental consent
- Harass, bully, or target individuals
- Distribute forms that promote violence, hate speech, or discrimination
Spam & Abuse
- Send unsolicited form links (spam)
- Use automated scripts to submit fake responses
- Abuse the free tier to circumvent plan limits
Legal Violations
- Collect sensitive personal data (health, biometric, financial) without appropriate legal basis and security measures
- Violate GDPR, CCPA, HIPAA, or other applicable data protection laws
- Infringe intellectual property rights
Enforcement
We may remove forms, suspend accounts, or terminate access without notice if we determine a violation of this policy has occurred. Serious violations may be reported to law enforcement.
We review reports of policy violations within 48 hours. To report abuse, email abuse@upperforms.com.
Appeals
If you believe your account was suspended in error, contact us at support@upperforms.com within 14 days with details of your situation.
